Through SharedHealth.com (the “Website”), Shared Health (“SH” or “we”) receives and maintains certain information regarding our subscribers, members, and guests. The information we collect from the Website depends in part on what you do when you visit or transact business on the Website. SHI respects the privacy of every individual who visits our Website. Therefore, we would like to define the types of information we receive and describe how it is maintained and protected in this Privacy Policy.

This Policy refers only to the information collected and maintained from the Website. This Privacy Policy supplements and is incorporated into our User Agreement, which, together with this Privacy Policy, govern your use of the Website. By using this Website, you confirm your consent and agreement to the User Agreement, including the Privacy Policy.

We Collect and How We Use It – We collect identifiable and non-identifiable information from you when you provide it to us, automatically as you navigate the Website, and from third-party service providers. Guests – Users of our Website often visit solely to obtain general information about SHI’s operations and services. If you visit our Website to read or download information, you use the Website as a guest. We use this information to measure the number of visitors to the different sections of our Website and to help us make our site more useful to visitors.

Registration – Physicians, brokers, and users who have health insurance coverage with SHI may be permitted to register to access a secured portion of our Website to conduct certain transactions. To register, SHI may ask for personal information (such as date of birth, social security number, name, address with zip code, telephone number, and email address) to verify appropriate your identity. You will be able to sign on and create a unique user name and password. We may also have access to your user name. If you sign up for these services, you will receive a written notification by mail, and if you did not register to access the services available on our Website, please contact SHI immediately by one of the methods on our Contact Us page.

In addition to the ways we’ve identified above, we use the information we have about you to facilitate the provision of services to you, improve our products and services, and for our own research. We may incorporate information you submit through the Website with our other records we have about you. In that case, it may be subject to state and federal laws about privacy. Sharing Your Information – We do not sell your personal information for commercial purposes. We may share your information with third-party service providers that help us run the Website or that work with us to facilitate the products and services we offer. Use and Disclosure of Protected Health Information – Some of the information you submit through the Website may be considered Protected Health Information (as defined by the Health Insurance Portability and Accountability Act of 1996 and its regulations (“HIPAA”)). We use, disclose, and protect this information in compliance with HIPAA and other state and federal laws or regulations (“Privacy Laws”). While on a secure page, such as the login page, the lock icon on the bottom of web browsers such as Microsoft Internet Explorer becomes locked, as opposed to unlocked, or open, when you are just “surfing.”

Third Parties – Your information may be held by third parties or we may link to other sites or applications not affiliated, owned, or controlled by us. Further, we may use products and services of third parties, for which the disclosure of your information is necessary, to help us better provide our services to you. We expressly disclaim any and all liability for the actions of third parties. The privacy practices of any third parties are outside the scope of this Privacy Policy and their use or disclosure of your information (including your personal information) will be governed by their own privacy practices or policies. Users Under 13 Years of Age – SHI often covers minor dependents through their parent or legal guardian’s health insurance policy. The Website is intended for general audiences and is not directed to children under 13 years old. Children under the age of 13 can access our site only as guests and we do not intentionally collect personal information from users under 13. If you become aware that your child has used this Website, and we may have personal information about a user under 13 from this Website, please contact us using one of the methods on our Contact Us page so we may delete the information.

Email – Many of our users want to receive information such as new product offerings, health education, and newsletters via email. If you give us your email address through the Website, we may provide this type of information electronically. SHI will also utilize email to perform surveys and/or gather other general information. Users that do not want to receive electronic information or surveys may opt-out using one of the methods set out in this Privacy Policy.

Email via the Internet may not be secure. If you send email to us directly, please do not include any sensitive information or health or claim-related information in your email correspondence. This includes Medicare numbers, Social Security numbers, personal medical information, or other confidential information. If you have a question about a specific claim, please call or write to our office.

Cookies – Cookies are small bits of electronic information that a website or application sends to your browser and are stored on your hard drive. We use cookies to identify users, keep the Website secure, and improve the Website’s performance. Cookies are not required to browse our Website and may be disabled or deleted. Personal Data Protection – SHI is committed to keeping your data safe from unauthorized access. You can help keep your personal information protected. SHI will never make unsolicited phone calls or send emails asking members to provide, update, or verify personal information, such as address, member IDs, Social Security numbers or other confidential information. If you receive a phone call or an email from a purported SHI representative asking for this information, please contact us at 1-844-474-6477, TTY 711 to report this incident.

There are additional steps that you can take to protect all your personal information including: (1) create difficult passwords for online access to your bank, bill payment and other web-based services, including Shared Health Access, (2) never share your passwords with anyone, (3) use caution when opening unsolicited emails and attachments, as computer viruses can be passed along via email, (4) immediately report lost or stolen cards, including credit cards and member ID cards, and (5) contact the national credit reporting agencies – Trans Union, Equifax, and Experian – if you feel that your personal information has been accessed or is at risk.

Opt-Out – In some cases, SHI provides its members and users the opportunity to ‘‘opt-out’’ of having their information used for purposes not related to the Website. Users who wish to opt-out can do so by contacting us using one of the methods on our Contact Us page. If you decide to opt-out, that decision will be effective until you revoke it by contacting us again.

Accessing, Updating, or Deleting Your Personal Information – If you would like to access, modify, or delete your personal information collected through the Website, contact SHI using one of the methods on our Contact Us page. We will promptly review all such requests in accordance with applicable laws.

Changes and Updates to Privacy Policy – SHI may change or update this Privacy Policy at any time, and this will be reflected by the date above.
