What’s the difference between HMO & PPO?

HMO plans must use providers that are in the network.

$0 Copay

PPO plans have the flexibility to use providers both in- and out-of-network without a referral. Members may need to file a claim if the out-of-network provider is unwilling to file the claim with the plan.

Do I need a Primary Care Provider (PCP)?

Yes

HMO plans require you to have a PCP – they can help coordinate your healthcare services.

No

PPO plans do not require you to have a PCP – you can see any provider you want. But, we encourage you to have a PCP – they can help coordinate your healthcare services.

If I have a doctor or a specialist who is out-of-network, will I still be able to see them and have the costs covered?

No

HMO plans don't cover care from out-of-network healthcare providers. There are a few exceptions including emergency or urgently needed services.

Yes

PPO plans give you the flexibility to visit providers outside of your network. However, when visiting an out-of-network provider, you may need to file a claim for reimbursement.

How to Qualify

Our Medicare Advantage plans are special types of plans known as Dual Eligible Special Needs Plans or D-SNPs. If you are eligible for Medicare and Medicaid, you may be eligible for one of our plans.
