Shared Health® Clinical Xchange® Connection™ gives you the tools you need to connect your clinical community. If you or the physicians you work with already use an EMR, there’s no need to change systems or upgrade to new hardware. As a web-based application, Clinical Xchange Connection is compatible with your current systems and interoperable with other systems throughout your community.

Clinical Xchange Connection, and the rest of the Clinical Xchange Suite, includes the Shared Health® Clinical Health Record® (CHR), a community-based patient-centric health record, accessible at the point of care, that enables clinicians to get a comprehensive, longitudinal view of a patient’s health.

The CHR consists of:

Patient Summary – Presents each patient’s information and records pertaining to their medical history, both in a clinician’s practice and beyond, including:

• Basic patient demographics
• Medical claims encounter data
• Medication history
• Allergy information
• Primary care physician contact information

Clinical Decision Support – Helps clinicians manage the care of all their patients by enabling more informed decision-making. Our clinical decision support consists of proactive patient management tools such as Problem Lists and Care Opportunities®.

• Problem Lists analyze clinical and administrative patient data, looking for key indicators and patterns that would suggest a patient potentially has a chronic condition. Problem Lists automatically notify clinicians at the point of care if there are conditions that a patient may have.
• Care Opportunities uses nationally recognized evidence-based clinical practice guidelines to give clinicians the ability to quickly recognize gaps in care, and take the appropriate steps to close those gaps.

Interoperability and Integration – Provides the standardized connectivity needed to link everyone in the community. We ensure data is seamlessly integrated with the systems practices and hospitals connect with.

We use a standards-based approach, including HL7, ANSI X12, NCPDP Script, and XML feeds. In addition, Shared Health Clinical Xchange supports system integration capabilities, including SAML federation, Single-Sign-On, web services, and APIs.

All information in the Shared Health CHR® is securely protected by industry-standard security safeguards, has strictly enforced role-based access controls, and is stored and transmitted in accordance with HIPAA privacy regulations.
