Already Registered?

If you already have a Shared Health User ID and Password, you may not need to register at this time. If you registered with Shared Health on or after 8/1/2008, you may log in at the top of this page. If not, please read the following instructions.

New to Shared Health?

To create a Shared Health account, you will first need to obtain a Personal Identification Number (PIN). To request your PIN, go to Step 1 below. After completing this step your PIN will be mailed to you. This will enable you to complete the registration process.

Returning Member?

If you are already a registered member of Shared Health and:

  • Know your PIN, click on Step 2 below to complete the registration process
  • Have forgotten your PIN, click on Step 1 below to have a new PIN mailed to you

FAQ

Why do I need to re-register?

To ensure the privacy and security of your personal health information, Shared Health cannot transfer your login credentials (user ID and password).

Why can’t I get my PIN over the phone?

For security purposes, we cannot give you your PIN over the phone. We will send it to the mailing address that we have on file for you.
